Prior to your visit

Thank you for choosing The Vision Center at Seaside Farms for your eyecare and eyewear needs. We ask that you please bring in all of your insurance cards, photo ID, and any glasses/sunglasses that you wear. In order to help us treat you effectively and in a timely manner, we kindly ask you to take the time to review all the information below and complete the required sections.  To make your visit more enjoyable and to provide you with the best possible exam, we will be performing a retinal screening, including OCT and Optomap, instead of dilating you. These images are typically not covered by your insurance and cost $49. Please be advised that masks are optional inside our office at this time.
